Client Acknowledgement:

  • I acknowledge by signing this agreement that I have been given the full opportunity to ask any and all questions which I might have about the obtaining of a tattoo and that all of my questions have been answered to my full satisfaction. I specifically acknowledge I have been advised of the facts and matters set forth below and I agree as follows:


  • I confirm that I am at least 18 years of age, meeting the legal age requirement for receiving tattoo services in Ontario, Canada.


  • I understand that I am responsible for full payment of the tattoo services as described in the pricing guide provided by JR Tattoos Inc.


  • I acknowledge that I have reviewed and approved the final design for my tattoo. Any changes to the design must be agreed upon by both parties.


  • I agree to follow the aftercare instructions provided by JR Tattoos Inc. to minimize the risk of infection and complications. I understand that failure to adhere to these instructions may lead to adverse reactions and complications for which JR Tattoos Inc. is not responsible.


  • I understand that the tattooing process involves risks, including but not limited to allergic reactions, infection, scarring, and variations in the final appearance of the tattoo. If I have diabetes, epilepsy, hepatitis, hemophilia, HIV aids, or any other communicable disease, heart condition or take medicine that thins the blood I have advised my tattooist.


  • I am not pregnant or nursing. I am not under the influence of alcohol or drugs. - I do not have medical or skin conditions. If I have any type of infection, rash, or lesion anywhere on my body, I will advise my tattooist.


  • I acknowledge it is possible I might have an allergic reaction to the pigments or processes used in my tattoo, and I agree to accept the risk that such a reaction is possible.


  • I acknowledge that infection is always possible as a result of the obtaining of a tattoo, particularly in the event that I do not take proper care of my tattoo.


  • I agree that any touch up work needed due to my own negligence or failure to follow aftercare instructions, will be done at my own expense.


  • I acknowledge that a tattoo is a permanent change to my appearance. To my knowledge, I do not have a physical, mental or medical impairment or disability which might affect my well-being as a direct or indirect result of my decision to have a tattoo.


  • I acknowledge and allow my tattoo to be photographed, recorded, and showcased on both physical and online business portfolios.


  • I acknowledge that security cameras are present on the property and that for safety precautions activities are monitored and recorded.


  • I acknowledge I have truthfully represented my tattooist that the obtaining of the tattoo is by my choice alone. I consent to the application of the tattoo.

Aftercare Instructions:

After Getting Tattooed


  • Leave the bandage on for 2-4 hours: Your tattoo artist will cover your tattoo with a bandage. Leave it on to protect against bacteria.


  • Wash your hands: Before touching your tattoo, make sure your hands are clean. - Gently remove the bandage: Peel it off slowly. If it sticks, wet it with warm water to ease removal.


  • Cleanse the tattoo: Use mild, fragrance-free soap and lukewarm water to gently clean the tattoo. Pat it dry with a clean, soft paper towel. ​ Healing Phase


  • Keep it clean: Wash your tattoo gently with mild soap and water, twice a day.


  • Pat dry: After washing, gently pat your tattoo dry with a clean, soft cloth or paper towel. Do not rub.


  • Apply a thin layer of ointment: Use a recommended tattoo aftercare ointment or lotion. Apply a thin layer, and don't overdo it.


  • Avoid direct sunlight: Protect your tattoo from the sun. If you must go outside, use sunscreen (SPF 30 or higher) after the tattoo is fully healed.


  • Avoid soaking: Avoid long baths, swimming, or hot tubs during the healing process. Short showers are fine.


  • Avoid tight clothing: Wear loose, breathable clothing to prevent irritation. ​ Final Stages of Healing - Continue cleaning and moisturizing: Keep up with your cleaning and moisturizing routine.


  • Expect peeling: Your tattoo may start to peel and itch. Do not scratch or pick at it.


  • Avoid excessive moisture: Be cautious about over-moisturizing; your tattoo needs to breathe. ​ Long-Term Care - Stay hydrated: Drinking plenty of water helps your skin stay healthy.


  • Protect from sun: Always use sunscreen on your tattoo when exposed to the sun.


  • Avoid harsh chemicals: Keep your tattoo away from products like alcohol, hydrogen peroxide, and chlorine.


  • Consult your artist: If you have any concerns or questions during the healing process, don't hesitate to contact your tattoo artist.

d. Pus or Excessive Drainage: Any unusual discharge, especially if it is yellow or green in color and has a foul odor, should be reported to a healthcare professional. This may be a sign of infection.


e. Unexplained Skin Changes: If you observe significant changes in the appearance of your tattoo, such as the development of large, raised bumps, persistent scabbing, or the tattoo becoming unusually hot to the touch, consult a doctor.

a. Infection Prevention: Prompt medical attention can prevent the progression of an infection, which, if left untreated, may lead to more serious health issues.

c. Pain Management: Healthcare professionals can provide pain management strategies and appropriate medications if necessary.


3. How to Identify Infection or Poor Healing:

a. Increasing Redness: If the redness around the tattoo area intensifies instead of fading over time, it may indicate a problem.


b. Swelling That Persists: Swelling should gradually reduce. If it increases or remains the same, consult a healthcare professional.


c. Pain That Gets Worse: Ongoing or worsening pain may suggest an issue.


d. Pus or Discharge: Any unusual discharge, especially if it has an unusual colour or odour, is a red flag.


e. Fever or Chills: The development of a fever or chills is a sign of infection and should be addressed promptly.

Introduction: Welcome to JR Tattoos Inc., operated by JR Tattoos Inc. ("we," "us," or "our"). These Terms and Conditions govern your use of our tattoo services, and by using our services, you agree to be bound by the terms outlined herein. Please read these Terms and Conditions carefully before proceeding with our services. If you do not agree with any part of these terms, please do not use our services.


Age Requirement: You must be at least 18 years old to receive tattoo services at JR Tattoos Inc. By booking an appointment and using our services, you confirm that you meet this age requirement.


Consultation and Consent: Before receiving a tattoo, you may book a consultation with our tattoo artist to discuss your design and any concerns you may have. You must provide accurate information about your medical history, allergies, and any preexisting conditions that may affect the tattooing process. You must also sign a consent form, acknowledging that you understand the risks associated with tattooing.


Deposits and Booking: A non-refundable deposit is required to secure your appointment. This deposit will be applied to the cost of your tattoo. By submitting a deposit, you confirm your commitment to the appointment and to these Terms and Conditions.


Tattoo Design Approval: You will have the opportunity to approve the final design before the tattooing process begins. Any changes to the design must be agreed upon by both you and our tattoo artist. Once the tattooing process begins, changes may not be possible, and additional charges may apply for modifications.


Health and Safety: We adhere to strict health and safety standards, including the use of sterile equipment, needles, and inks. However, it is essential that you follow our aftercare instructions carefully to minimize the risk of infection and complications. We are not responsible for any adverse reactions or infections that may occur due to failure to follow our aftercare guidelines.

Cancellation and Rescheduling: If you need to reschedule your appointment, please provide at least 48 hours' notice. Failure to do so may result in the forfeiture of your deposit. In the event that we need to reschedule your appointment for any reason, we will make every effort to accommodate your schedule.


Pricing and Payment: Our pricing is determined by the size, complexity, and time required for your tattoo. You will receive an estimate before your appointment. Full payment is due upon completion of the tattoo. We accept cash, debit and major credit cards as payment methods.


Intellectual Property: The designs created by our artists are protected by copyright. You may not reproduce, modify, or distribute any designs without our explicit written consent.


Liability Waiver: By receiving tattoo services from JR Tattoos Inc., you acknowledge that tattoos involve certain risks, including but not limited to allergic reactions, infection, and scarring. You release JR Tattoos Inc., its artists, and employees from any liability for any injuries or complications that may arise as a result of the tattooing process.


Dispute Resolution: Any disputes or claims arising from our services will be subject to the laws of Ontario, Canada. You agree to resolve any disputes through arbitration or small claims court, and you waive any right to pursue a class action lawsuit against us. In the event of a dispute, if you or anyone representing you chooses to dispute these terms and conditions in court, you or your representative will be responsible for paying all legal fees incurred by JR Tattoos Inc.


Modifications: We reserve the right to modify these Terms and Conditions at any time. Any changes will be posted on our website, and it is your responsibility to review them periodically.


Consent Agreement: I hereby release and forever discharge and hold harmless the tattooist from any and all claims, damages or legal actions arising from or connected in anyway with my tattoo, or the procedure and conduct used in performing my tattoo, to the fullest extent allowed by the law. I release JR Tattoos Inc., its artists, employees, and affiliates from any liability for injuries, complications, or adverse reactions arising from the tattooing process. I understand that the tattoo artist will use sterile equipment, needles, and inks to minimize health risks.


Any disputes or claims arising from the tattoo services will be subject to the laws of Ontario, Canada. I agree to resolve any disputes through arbitration or small claims court and waive any right to pursue a class action lawsuit. I have read and understood the above terms and conditions. By choosing to reply to complete this form, I hereby confirm my consent to receive tattoo services from JR Tattoos Inc. and my agreement to abide by the terms outlined in this document.
